USER ID Instructions

• USER ID - Your User ID must be an email address. Your email address is used to send automated reminders for onsite courses and to set up your Course Profile. Please do not set up multiple User IDs by using different email addresses. If you do not remember your User ID, please contact us at fcphpinfo@health.usf.edu.
